Case Management

Our case managers help people living with HIV navigate the sometimes overwhelming world of service providers. Having a case manager by your side can help alleviate stress and confusion around staying healthy, meeting your needs, and receiving care while living with HIV. Your case manager is your expert guide, and can help with landlords, insurance companies, pharmacists, and medication support programs. They can also help you navigate the Wyoming Department of Health’s financial support programs for people living with HIV. Access to Health Care and other Supportive Services

The Wyoming Department of Health’s Communicable Disease Treatment Program provides persons living with HIV/AIDS access to comprehensive health care and other supportive services. These programs serve as the payer of last resort for persons who are uninsured, under-insured, or are otherwise unable to access medical treatment. Funding for these programs is provided, in part, by grants from the United States Department of Health and Human Services, Health Resources Services Administration, and the United States Department of Housing and Urban Development. Some of the available services provided by the programs include:

  • Mental Health and Substance Abuse Treatment
  • Nutritional Services
  • Case Management Services
  • Insurance Premium Assistance

PROGRAM ELIGIBILITY REQUIREMENTS:

In order to be eligible for assistance, an individual must meet all three of the following requirements.

You:

1. Must be HIV-positive or have a diagnosis of AIDS.

2. Must be a resident of the state of Wyoming.

3. Must have a valid photo ID.

Not much data is available on COVID-19 and people with HIV but based on what is known, it does not appear to affect people with HIV differently than other people. However, since there is not definitive evidence to support this and until more is known, people with HIV, especially those who are older and/or who have advanced HIV or poorly controlled HIV, as well as other conditions like heart or lung disease or diabetes, should be extra cautious.

Based on how people with HIV are affected by other health issues targeting the lungs, those who are not on HIV treatment, or who have advanced HIV disease (a low CD4+ T cell count - generally less than 200/mm3) may be at higher risk for getting sicker from COVID-19. Older people with HIV and those with other conditions like heart or lung disease or diabetes also may be at higher risk for getting sicker if they have COVID-19. 

Many people living with HIV in the U.S. are over 50 years of age. If you know one of these people, please check in with them around their mental and physical health in this trying time. Though these are undoubtedly resilient folks, this can be a trying time of social and physical isolation for us all. Offer to get them groceries, ensure they have the prescriptions they need, and if they need further resources, please contact us.
